Mechanism of Action

Gonadorelin is a synthetic decapeptide (pGlu-His-Trp-Ser-Tyr-Gly-Leu-Arg-Pro-Gly-NH2) identical in sequence to endogenous gonadotropin-releasing hormone (GnRH), also known as luteinising hormone-releasing hormone (LHRH). GnRH is the central regulator of the hypothalamic-pituitary-gonadal (HPG) axis: produced by hypothalamic neurons in the preoptic and arcuate nuclei, GnRH is released in pulsatile fashion into the hypophyseal portal system and travels to the anterior pituitary, where it binds the GnRH receptor (GnRHR) on gonadotrope cells. GnRHR is a class A G protein-coupled receptor that activates Galphaq/PLCbeta signalling, producing IP3-mediated calcium release and DAG-mediated PKC activation. This signalling cascade stimulates the synthesis and release of luteinising hormone (LH) and follicle-stimulating hormone (FSH) from gonadotropes. LH and FSH then act on the gonads (testes and ovaries) to regulate steroidogenesis (testosterone, oestrogen, progesterone production) and gametogenesis (sperm and egg production). The pulsatile pattern of GnRH release is critical: continuous (non-pulsatile) GnRH receptor activation paradoxically suppresses gonadotropin release through receptor desensitisation, a phenomenon exploited clinically by GnRH agonist desensitisation protocols.
